Plan Description For smart coverage at sensible savings, these plans come in a wide rage of deductible/monthly premium combinations designed to give you lots of affordable options. Whichever plan you choose, you'll have coverage for your first three network doctor visits including routine office visits - without having to meet your deductible first. These are great plans packed with benefits including low copays for doctors' visits, plus comprehensive prescription drug coverage. Premier plans offer a wide range of deductible/monthly premium combinations. And routine preventive benefits are not subject to your plan's deductible. Optional maternity benefits are avalable on select plans, too. The Lumenos HSA plan has two parts: A high-deductible health plan (HDHP) and a Health Savings Account, or HSA. An HSA is like a savings account for your health. You contribute funds to your HSA to save for eligible medical expensees. Your HSA contributions may have potential tax advantages and any unuses dollars are rolled over from year to year for future health expenses. The Lumenos HIA Plan has two parts: A high-deductible health plan (HDHP) and a Health Incentive Account or HIA. An HIA is like a savings account for your health. Your HIA is funded by reward credits that you earn by participating in Health Incentive Programs. You then use these funds to pay for eligible medical expenses. Unused funds are rolled over from year to year to help you pay for future medical expenses. The Lumenos HIA Plus Plan has two parts: A high-deductible health plan (HDHP) and a Health Incentive “Plus” Account, or HIA Plus. An HIA is like a savings account for your health. You use the funds from your HIA to pay for eligible medical expenses. With HIA Plus, your Health Incentive Account is funded in two ways: 1) Anthem makes quarterly contributions. 2) You can earn reward credits by taking steps to improve and maintain your health. Any unused funds are rolled over from year to year to help you pay for future health expenses.
